I think it makes sense separating problem understanding from solution. Both together overwhelm people. First we have to precisely understand the problem, then explore solutions.

Regarding estimates, you're totally right. If they are often inaccurate, it means the refinement are not being deep enough. It's a sign to improve the approach.
